  • a power of attorney, or

  • a guardianship court decision

• Medical documentation, especially:

  • medical reports

  • hospital discharge summaries

  • GP or specialist recommendations

⚠️ Note: Medical documentation is not strictly required by law, but without it the application may be rejected for “insufficient proof of health condition.”

Recommended supporting documents:

• Statement from the treating physician – brief summary of health status and care needs

• Statement from the caregiver – can clarify the scope of care in practice

• Photo documentation – in cases of mobility issues or home adaptations

📝 HOW TO FILL OUT THE FORM

The form “Application for Care Allowance” is available:

• at the Labour Office (ÚP)

• online via the Ministry of Labour and Social Affairs (MPSV)

• or as an electronic form (recommended)

Common mistakes:

• ❌ missing date of birth or permanent address

• ❌ unsigned application

• ❌ caregiver section not filled in

• ❌ vague or generic descriptions (e.g. “daily care”)
